Social Prescribing - Community of Practice Gathering reservation

One of my first ventures out as Wellbeing Lead for DVSC was to attend a Social Prescribing event at Nant Gwrtheyrn in Pwllheli, Gwynedd. The event just happened to fall on National Social Prescribing Day, with the focus being to further develop the community, our approach, and the collective change we want to drive.

Social Prescribing enables health professionals refer patients to support provided in their community, such as local voluntary organisations and community run activities, in order to improve their health and wellbeing. It also aspires to support individuals to take greater control of their own mental health and physical wellbeing. Social Prescribing can also be referred to as community referrals, community navigation and community connectors.

There are many different models for social prescribing, but most involve a link worker or navigator who works with people to access local sources of support.

The event brought together people from health, local authority, and voluntary, community and social enterprise sectors to explore the benefits of social prescribing. Various organisations spoke at the open mic session – Social Prescribing in Practice, all spoke of the benefits and ways to mainstream social prescribing in a sustainable way.

One of the biggest success stories is Vale of Clwyd Mind, a member of DVSC who have just launched a new Social Prescribing service which aims to improve well-being and quality of life for people living in Rhyl. The project, which is funded by the Welsh Government as part of an evaluation study, has been specially created for adults with mild to moderate or emotional problems. The project is run by two fully trained and experienced Link Workers, and it can now be accessed through all GP surgeries in Rhyl. For further information please click here.

One of the advantages of being a member of Third Sector Support Wales is the opportunity DVSC has to celebrate, learn from and share best practice from our pan Wales’s network.

Presgripisynu Cymdeithasol

Un o fy ymweliadau cyntaf fel Arweinydd Llesiant Cyngor Gwasanaethau Gwirfoddol Sir Ddinbych (CGGSDd) oedd mynd i ddigwyddiad Presgripsiynu Cymdeithasol yn Nant Gwrtheyrn ym Mhwllheli, Gwynedd. Roedd y digwyddiad yn digwydd cael ei gynnal ar Ddiwrnod Cenedlaethol Presgripsiynu Cymdeithasol, ac roedd yn canolbwyntio ar ddatblygu’r gymuned ymhellach, ein dull a’r newid rydym ni am ei sicrhau ar y cyd.

Mae Presgripsiynu Cymdeithasol yn galluogi gweithwyr iechyd proffesiynol i gyfeirio cleifion at gymorth a ddarperir yn eu cymuned, megis mudiadau gwirfoddol lleol a gweithgareddau a gynhelir gan y gymuned, er mwyn gwella eu hiechyd a’u lles. Mae hefyd yn ceisio cefnogi unigolion i gymryd mwy o reolaeth dros eu hiechyd meddwl a’u lles corfforol eu hunain. Weithiau cyfeirir at bresgripsiynu cymdeithasol fel atgyfeiriadau cymdeithasol, llywio’r gymuned a chysylltwyr cymunedol.

Mae llawer o wahanol fodelau presgripsiynu cymdeithasol, ond mae’r rhan fwyaf yn cynnwys gweithiwr cyswllt neu lywiwr sy’n gweithio gyda phobl i fanteisio ar ffynonellau cymorth lleol.

Gwnaeth y digwyddiad ddod â phobl o’r sectorau iechyd, awdurdodau lleol a grwpiau gwirfoddol, cymunedol a mentrau cymdeithasol ynghyd i ystyried manteision presgripsiynu cymdeithasol. Bu gwahanol fudiadau’n siarad yn y sesiwn meicroffôn agored – Presgripsiynu Cymdeithasol ar Waith, ac roeddynt i gyd yn siarad am y buddion a’r ffyrdd i brif ffrydio presgripsiynu cymdeithasol mewn modd cynaliadwy.

Un o’r llwyddiannau mwyaf yw Mind Dyffryn Clwyd, aelod o CGGSDd, sydd newydd lansio gwasanaeth presgripsiynu cymdeithasol newydd sy’n ceisio gwella llesiant ac ansawdd bywyd pobl sy’n byw yn y Rhyl. Mae’r prosiect, a gyllidir gan Lywodraeth Cymru fel rhan o astudiaeth werthuso, wedi’i greu’n arbennig ar gyfer oedolion gyda phroblemau emosiynol ysgafn i gymedrol. Arweinir y prosiect gan ddau Weithiwr Cyswllt profiadol, wedi’u hyfforddi’n llawn, a gall yr holl feddygfeydd yn y Rhyl ei ddefnyddio yn awr. Effaith presgripsiynu cymdeithasol

Os ydych chi’n llunio achos busnes o blaid presgripsiynu cymdeithasol, darllenwch yr adolygiad hwn sy’n edrych yn feirniadol ar dystiolaeth effaith economaidd presgripsiynu cymdeithasol.

Un o fanteision bod yn aelod o Gefnogi Trydydd Sector Cymru yw’r cyfle sydd gan CGGSDd i ddathlu, dysgu oddi wrth a rhannu arferion gorau gan y rhwydwaith sy’n cwmpasu Cymru gyfan.
